Job summary Chesterfield Royal Hospital is looking to appoint a fully qualified or an existing trainee ACP to join our established, innovative, and improvement-focused acute frailty team The overriding purpose is to provide early Comprehensive Geriatric Assessment to those who need it most. The successful applicant will use their advanced clinical skills to support the provision of the highest quality patient care through personal actions and aiming for continuous improvement.
Core components of the role will be ability to deliver education, provide leadership and become involved in service development once established in the post.


Main duties of the job To provide high quality clinical assessment, intervention, and treatment for a wide variety of older people living with frailty. Early identification of those requiring Comprehensive Geriatric Assessment and initiation of the process by becoming a core member of the Acute Frailty multidisciplinary teams. Using their expertise in frailty to advocate for and support the patient journey.
Working closely with the Acute Frailty consultants.


To work independently at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions based on advanced clinical skills assessment.


The post holder will demonstrate safe clinical decision making and expert care including assessment and diagnostic skills for patients.


The post holder will provide clinical leadership and support to improve the patient experience and provide safe, quality care.


To support junior medical staff sharing clinical workload and allow education and training requirements to be met.


The post holder will work collaboratively with clinical teams to meet the needs of patients, supporting the delivery and development of policy and procedures and providing appropriate leadership as required ensuring the maintenance of clinical excellence.
